How do I get my physician to fill out a prescription ?

Simply provide our fax number to your physician for prescriptions to be sent or ask your physician’s office to look us up on its electronic prescribing platform by name, phone, or zip code.

How does the mail order system of Midwest Specialty Pharmacy Work?

We will work directly with your doctors office to the get your prescription sent over to us, with your permission. We can also transfer previous prescriptions from an existing pharmacy. Once we receive the referral from your doctor, we will resolve any denials or questions with your insurance provider, and contact you to enroll into automatic medication delivery, should you choose.

Medication will be delivered directly to your door through expedited shipping within 1-2 business days on average.

You will never be stuck without your medications or have to drive to your local pharmacy and wait in long lines.

Can I transfer prescriptions from another pharmacy to Midwest Specialty Pharmacy?

Yes. Let us know which prescriptions you need transferred and which pharmacy you have been using. As long as there are remaining refills for your medication, Midwest Specialty Pharmacy will fill them and deliver them to your home with the same affordable service.

What if my physician prescribes medications that are not covered by my work injury or accident?

Your provider must be prescribing the medication for a condition caused by the work injury or accident. Otherwise, we will be unable to provide you the medication without an out of pocket charge.  

What if I am unable to find a physician to provide a prescription??

If you have an attorney who represents you on your claim, you may want to seek guidance with them regarding your injuries and course of care. If you would like the name of a physician or an attorney who handles work injury or personal injury claims, you may check your public listings, or contact us.

What do I need to do once I have a prescription?

Option 1: Electronic Prescribing by your provider

Option 2: Fax & Mail ( This needs to be done by your provider or physicians office)

Step 1: Make sure all prescriptions are dated and signed by the authorizing medical provider.

Step 2: Have the provider fax ALL prescriptions and a patient information form to 312-690-4585.

Step 3: Determine if any of the medications are Schedule II Controlled Medications. If your physician is NOT prescribing any, nothing further is needed. If he or she is prescribing any, mail ALL of the original prescriptions and a patient information form to the following address:

What is a controlled medication, and how do I know if a prescribed medication is controlled?

A controlled medication is a drug or chemical whose manufacture, possession, or use is regulated by the government.

Am I able to get my prescriptions refilled through your pharmacy?

Our team will make sure you receive your refill in a timely matter every month after you enroll in the automatic delivery service with our pharmacy. This service is optional and may or may not be for everyone and is available for all prescriptions with approved refills.

Our pharmacists will call you to provide directions for new medications and follow up on the course of you recovery. You are always a phone call away, should you have any questions or concerns about your treatment. Call us at 1-800-864-8383.

If your prescription does not have a valid refill, you may contact your provider or you can request that we contact them on your behalf.

Will my group health insurance be billed?

No we will not bill your group health insurance.

Will my PIP policy be billed?

Only if this is authorized by yourself, your prescribing provider or legal representative.

Will my MedPay policy be billed?

Only if this is authorized by yourself, your prescribing provider or legal representative.

Can I sign up for Midwest Specialty Pharmacy from any state?

Midwest Specialty is currently licensed to deliver medications to 7 states. Some states may not be eligible at this time.Licensed states ->

Do I need to be at home to receive my package / medications?

Unless you are prescribed controlled medications (Controlled Substances CII-CV), you will not have to be at home at the time of the delivery. You may opt to have signature confirmation on all packages regardless of whether the medication is controlled or not. We use common carriers such as UPS, Fedex, and USPS.

Controlled medications will require your signature upon delivery. If you happen to be away from home at the time of the delivery, our delivery vendor will make two more attempts to deliver the medication before returning the package to our pharmacy. If you have any concerns about deliveries please contact us with any questions.

Will a tacking number be provided for my package?

At the time of the first prescription you fill with us, you may request the tracking number be emailed or texted to you at each fill of a prescription on file.

Does the Pharmacy or do I have to obtain pre-authorization before medications are delivered?

No, pre-approval is not required, however, in some situations we may need to contact your physician or insurance carrier.

What happens if I have received medications from Midwest Specialty Pharmacy and it is not paid for by my workers' compensation insurance?

If you claim is denied or litigated, Midwest Specialty Pharmacy can continue to fill and ship medications to you if you have a legal representative like a Workers’ Compensation Attorney. You will not be charged as long as your claim is still active.

If you do not have a legal representative, our pharmacy will inform you in writing of this, at which time, no additional medications not paid will be dispensed unless confirm with valid attorney representation, that a valid claim is pending and that your unpaid bills will be protected at the time of settlement.

Who pays for my medication?

We will bill your workers’ compensation insurance carrier for the prescriptions that Preferred Prescriptions fills for you. Workers' compensation claims generally do not have a copay. If you are a personal injury patient we may have to send a lien to your insurance carrier or legal representative.

Can medications be delivered to an office or another location or a PO Box?

Medications can be shipped to a work address or alternative address as long as someone is there to sign for the delivery. Medications CANNOT be shipped to a P.O. Box.

Who is eligible to sign for the medications upon delivery?

Any member of your family can sign for your package. You may request that only family members over the age of 18 sign for your package.

What happens if nobody is home to sign for the medications?

Any member of your family can sign for your package. You may request that only family members over the age of 18 sign for your package.

What happens if nobody is home to sign for the medications?

UPS and FedEX will leave a note on your door when a missed delivery occurs. When this happens, the common carrier will generally attempt to redeliver the medications the next day, with a total of two additional attempts. You may call the number of the door tag and request that you pick it up at a location near your home.

Who pays for the cost of shipping?

Our pharmacy will cover the cost of shipping and it will be included in the invoice we send to the insurance carrier and/or your attorney.  That is our guarantee.
