Sending Prescriptions Is Easy!

  • Option 1: Electronically Prescribing

    Look us up on your electronic prescribing platform by name, phone, or zip. Midwest Specialty Pharmacy, 312-690-4585, 60505
  • Option 2: Fax & Mail

    Step 1: Make sure all prescriptions are dated and signed by the authorizing medical provider.

    Step 2: Fax ALL prescriptions and the completed patient information form to 312-690-4585.

    Step 3: Determine if any of the medications are Schedule II Controlled Medications. If you are not prescribing any, nothing further is needed. If you are prescribingSchedule II Controlled Medications use our prepaid UPS Next Day envelopes to mail us the original prescription along with the completed patient information form to the following address:
